Creativity Release

I hereby irrevocably grant BlueDot and its successors, transferees, licensees and assigns permission to record, edit and make use of my image, voice, biography and/or personal experiences in BlueDot products and promotional material, in perpetuity, throughout the universe, in any way, by any means and in any media now existing or to be created, without any further compensation.

I understand that I should not share anything I want to keep private. I understand that once recording has finished, I will have no rights to or ownership interest in what is recorded.

I waive any right I may later have to complain that the exercise of rights under this agreement violates copyright, violates my privacy, defames me, puts me in a false light, or otherwise injures me.

I am NOT employed by BlueDot Productions and is NOT obligated to follow BlueDot instructions on what to do or not do during recording. I understand that I may stop recording at any time.

Any controversy or claim arising out of or relating to this agreement, or the breach thereof, shall be settled by arbitration administered by the American Arbitration Association in accordance with its Commercial Arbitration Rules, and judgment on the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof. In no event may any action be taken that impedes commercial exploitation of BlueDot’s products.

Parent/Guardian Consent:

As parent or legal guardian of the above-named person, I hereby irrevocably consent to the forgoing grant of permission. I agree to indemnify and hold BlueDot Productions harmless from any consequences of disaffirmance or rescission of the above.
